Stay Connected to Care

Get support managing your health with Sanford CareSignal. This program helps you stay in contact with your care team between appointments.

Each week, you will receive automated phone calls or text messages to check in with you and monitor your health. If you respond that your symptoms are not improving, a care team member will follow up or connect you with additional support.

Sanford CareSignal helps monitor:

  • Chronic obstructive pulmonary disease (COPD)
  • Depression
  • Type 2 Diabetes (noninsulin dependent diabetes)
  • Heart failure
  • Recovery after discharge from the hospital or emergency department

Participating in Sanford CareSignal only takes a few minutes each week, and you can pick a time to be contacted that works best for you. You must respond at least once each month to stay active in the program.

How to enroll
If you are eligible for the Sanford CareSignal program, a team member will call you to help you enroll.

Sanford CareSignal does not replace any communications you already have with your care team. Call 911 if you are having an emergency or 988 for the National Suicide and Crisis Lifeline.
