PACE

Our Program of All-Inclusive Care for the Elderly, also known as PACE, serves adults who qualify for nursing home care and older adults with physical, intellectual or developmental disabilities. PACE is a joint Medicare/Medicaid program that provides comprehensive medical and long-term care, as well as prescription drug coverage.

As a PACE member, you work with a team of dedicated healthcare professionals to coordinate all aspects of your care. Your team includes a Community Care primary care physician, social worker, nurse and several other professionals.

No Copays, Deductibles, or Insurance Gaps

We provide medical care, long-term care services, and prescription drugs.

You could be financially responsible for care services received from out-of-network providers that were not approved.

Active Involvement

We encourage active involvement from you, your family, and authorized representatives in determining the services and support that best meets your needs.

Manage Your Own Services
