If your agency's name does NOT appear as a selection in the drop-down list for the 'Organization Name' field of the form below, your agency does not have a PDA LMS 'organizational' account. If so, you can request guest access, which provides access to limited/publicly available training offerings, currently the PS Mandatory Abuse Reporting Training Course. CLICK HERE to establish a PDA LMS Guest Account*. If you desire access to all training course/resource offerings, your agency's Owner, President, Director, or Lead Personnel will need to submit an online request form to establish a PDA LMS 'organizational' account.

*Guest accounts are reserved for non-Area Agency on Aging (AAA) staff.

Area Agency on Aging (AAA) Staff: If you are staffed by an AAA agency, DO NOT use the link above to request a 'Guest Account' or DO NOT complete/submit the 'User' registration form below. Instead, consult with one of your agency's PDA LMS 'org admins'. Each AAA has at least two PDA LMS 'org admins' responsible for setting up training accounts for staff and volunteers under the agency's PDA LMS 'organizational' hierarchy.


PDA LMS 'USER' TRAINING ACCOUNT REGISTRATION FORM:

If you are staffed by an agency established as a PDA LMS 'organization', scroll down and complete the form below to request a PDA LMS 'User' training account. To determine if your agency is established as a PDA LMS 'organization', review the drop-down list below under the field titled 'Organization Name'. If your organization's name appears in the list, you may proceed to complete/submit a new user request form. If your agency does not appear in the list, please complete a 'guest' user account request using the link provided above, under the heading 'PDA LMS 'GUEST' ACCOUNTS'.

  • Only one (1) PDA LMS training account is authorized per individual. If you currently have an existing training account, please do not submit a new account request form. If you are unable to access your existing account, you may need to reset your login password or if the account is inactive due to non-activity for a period of three (3) or more years, you will need to submit a ticket to the PDA LMS Help Center requesting reactivation of your account.
  • If you transfer from one PA aging network agency to another, please DO NOT complete this form, but rather complete and submit a PDA LMS Help Center ticket to request the transfer of your existing account to your new agency.  When completing the help center ticket, under the 'Request Type' section, select the option that reads 'I transferred to a new agency and would like my existing training account reassigned from old to new agency.'
  • Accounts with no activity for a period of three or more years automatically become inactive. Should your account become inactive, please complete and submit a PDA LMS Help Center ticket to request reactivation of your account.
